POSITION:

1/2 FTE Consumer Directed Care Coordinator/
1/2 FTE Transportation Voucher Program Coordinator

This is a full time position with full benefits. The position is located in Cheyenne and consists of work in two different programs, consisting of approximately 20 hours per week in each program.


Consumer Directed Care Coordinator


DESCRIPTION:

The Consumer Directed Care Coordinator is responsible for completing the services necessary to ensure consumers with disabilities, who are eligible for the Wyoming Medicaid Long Term Community Home Based Waiver, are capable of directing their own personal care program and are receiving the necessary services to remain in a least restrictive environment.

Coordinators will also be responsible for outreach and education as to the issue of abuse, neglect, and exploitation of vulnerable adults.

The CDC Coordinator shall be responsible for compliance with the Wyoming Department of Health, Aging Division requirements for the Consumer Directed Care Program.

The CDC Coordinator will be responsible for the CDC Program in their designated service area. The Coordinator will be responsible for initiating and maintaining professional contacts with nursing home and hospital administration, hospital discharge planners, social workers, rehabilitation staff, home health agencies, public health nursing departments, meals on wheels, housing authorities/agencies and other community service agencies. The CDC Coordinator will work with Project OUT Specialists and hospital discharge planners to assess consumers leaving hospitals to determine their needs, and where appropriate, introduce the consumer to Consumer Directed Care.


DUTIES:

  • Assessing the consumer to determine consumer needs and ability to direct their own personal care attendants

  • Educating consumer on hiring, training, and managing their own personal care attendants

  • Developing consumer plan of care to be forwarded to the Aging Division for approval

  • Conducting monthly in home visits with consumer to determine appropriateness of care provided

  • Providing mandatory paper work to the CDC Program Manager on a timely basis in compliance with the Aging Division requirements

  • Promotes interagency collaboration; provides educational information/public presentations concerning Consumer Directed Care to health care providers, general public, local and state community leaders.

  • The CDC Coordinator will coordinate with other WILR Transition and IL Specialists to serve consumers who will be transitioned into communities in their service area.

  • Other duties as assigned.


    Transportation Voucher Program Coordinator


    DESCRIPTION:

    The Transportation Coordinator (TC) serves as primary direct service staff for the WILR Transportation Voucher Program, located in Cheyenne serving Laramie, Albany, Carbon, Platte, Goshen, and Niobrara Counties. The (TC) conducts consumer interviews and provides Transportation Voucher Program information; determines consumer eligibility for the program; with the consumer, develops a mileage plan; and educates the consumer on the voucher program and how to successfully utilize the program for improved access to transportation. The TC coordinates with the program bookkeeper and the consumer to ensure that the consumer is utilizing the vouchers in an appropriate manner, in accordance with the individual consumer’s mileage plan.

    The TC is responsible for marketing the Transportation Voucher Program in his/her service area and working with all transit providers to ensure that the program is being successful for the consumer and the transit provider. The TC is responsible for working with existing public transit providers, human service agencies, state departments, and consumers to initiate a community/regional transportation advisory council. The council will determine how transportation services can be coordinated and improved to better serve consumers with disabilities.


    DUTIES:

  • Consumers with disabilities and with a barrier to transportation are provided a transportation packet. The packet includes a release of information which must be completed by the consumer; an application for transportation voucher assistance; a seven day travel log which consumers must complete to determine how many miles they may utilize on the voucher program; and the rules and requirements of the transportation voucher program.

  • A Transportation Coordinator works with the consumer to ensure that all appropriate information is completed; that the consumer has a complete understanding of the program, and that the consumer is capable of appropriately utilizing the transportation vouchers. In addition, the Transportation Coordinator works with the consumer to determine transportation needs beyond the seven day travel log. Travel requirements on a monthly/quarterly basis are determined for routine trips, such as medical appointments, especially trips to appointments outside of the consumer’s local community of residence.

  • The Transportation Coordinator and the consumer develop a mileage plan based on the mileage which is required for a three month period (quarter). The mileage plan is signed by the Transportation Coordinator and the consumer. A book of numbered vouchers, similar to checks in a check book, are then provided to the consumer.

  • Riders’ use of vouchers/miles is recorded on a monthly basis. Riders who utilize more miles than allocated in their quarterly mileage plans will be contacted to determine the reason for additional costs. Additional miles which can not be justified may result in the consumer/rider losing access to the transportation vouchers/program. Riders who know that they will need additional miles during a quarter, should contact the Transportation Coordinator to make appropriate changes in their mileage plan.

  • The TC will work with senior centers and other transit providers to develop and manage a ledger system which can be utilized in place of the individual transportation checks.

  • Completes other duties and special projects as required.

  • The Transportation Coordinator is responsible to the WILR Executive Director .

  • Position will require travel, extensive at times.


    QUALIFICATIONS:

    1. College degree or equivalent experience in the provision of services to consumers in a social service program.

    2. Knowledgeable of interview skills, interpersonal relations as applied to interaction with consumers, agency, and community. Ability to work with a population from diverse social and economic backgrounds. Ability to identify individual needs and utilize/develop community resources to address those needs.

    3. Knowledgeable about community resources for assistance to people with medical, and/or training and counseling needs.

    4. Should possess ability to communicate effectively both orally and in writing; negotiate agreements and build partnership with a wide range of individuals and organizations; utilize web-based and PC software and e-mail to develop reports and document services provided; and handle multiple simultaneous tasks within time constraints.

    5. Understand and support the independent living philosophy.

    6. Self-starter, resourceful, ability to work independently and ability to communicate with consumers, peers, and work with a team to achieve consumer and agency goals and project objectives.

    7. Should possess or demonstrate the ability to acquire a thorough understanding of transportation issues of consumers with disabilities, existing transit agencies, and all local, state and federal programs which can provide transportation to their clients.

    8. Personal knowledge of disability preferred.

    9. Valid drivers license is required, with a personal vehicle for transportation as a requirement of the position.


    SALARY:

    $25,625.00 with full benefits
