Project OUT Transition Specialist

Description:

Serves as primary direct service staff for Project OUT in a designated service area. The Transition Specialist performs comprehensive consumer interviews in nursing homes and in hospital settings, extensive person centered and futures planning; brokers and coordinates a wide range of community-based long-term services and supports; serves as an advocate in guiding consumers with disabilities through a complex and fragmented service delivery system; and ensures expedited delivery of services to meet consumer needs to move back into or remain in the community.


Duties and Responsibilities :

• Provide comprehensive consumer interviewing and information gathering and perform extensive person-centered assessment(s) that include: functional, financial, social supports, health status, family supports, mental/cognitive status, risk identification, and consumer goals.

• Provide futures planning which includes: encouraging participant in the planning process and learn skills and techniques that may ultimately enhance their ability to live independently or in a supported living environment; finding non-traditional supports for independent living; and financial planning.

• Meet with or contact lead staff designated by nursing home or hospital partner agencies for input on person-centered plan development.

• Develop, coordinate and implement person-centered care plans which address: living arrangement, employment, daily routine, environments, participation in community, confidentiality issues; privacy concerns; degree of service and life satisfaction, consumer rights, safety, fair treatment, health and wellness, abuse and neglect, continuity, security and connection to natural support networks.

• Meet with or contact lead staff designated by the nursing home or hospital partner agencies to ensure plan implementation and serve as broker to coordinate all available community resources needed to address supports called for in plan.

• Meet with or contact Protection and Advocacy Attorney as needed to address and overcome legal barriers to plan implementation.

• Assist in the development and implementation of the Project OUT program.

• Communicate frequently with community human resources agencies and assist individuals in finding non-traditional and traditional supports for independent living.

• Accompany the consumer to agency appointments and assist consumer with the completion of service applications and other related documentation.

• Periodically review and follow-up on consumers to ensure service linkages and assure all aspects of service delivery are provided in a culturally competent and sensitive manner.

• Meet on a weekly basis with Project OUT Manager to coordinate services.

• Completes other duties and special projects as required.

• The Transition Specialist is responsible to the WILR Project OUT Manager and will coordinate with the Manager to develop and maintain Project Out.

• Position will require travel, extensive at times.


Knowledge, skills and abilities required for the position:

  • Required knowledge: Should possess or demonstrate the ability to acquire a thorough understanding of all local, state and federal health and human services programs with an emphasis on programs which serve the elderly and persons with disabilities of all ages.

  • Skills and Abilities: Should possess ability to communicate effectively both orally and in writing; negotiate agreements and build partnership with a wide range of individuals and organizations; utilize web-based and PC software and e-mail to develop reports and document services provided; work as part of a high performance team of professionals, and handle multiple simultaneous task within time constraints. Should possess an understanding and support of the independent living philosophy

  • A college degree and experience in the provision of services to consumers in a nursing and/or social service program is required. Experience may be substituted for education on a year-to-year basis.

  • Must have a valid driver’s license and a vehicle for transportation. It is necessary to be able to travel in the service area on agency business.

  • Must have an understanding of commitment to the concepts and philosophy of independent living and self-determination of persons with disabilities.

  • Personal knowledge of disability preferred.
