Partnerships to Independence - Project Out

A Program to Help You Move Back into the Community

Project Out is designed to assist individuals who want to return to independent living within the community.

Project Out will ensure that services and supports are provided to maintain the highest quality of life for an individual returning to the community

Project Out IL Transition Specialists will work with nursing home administrators, social workers, discharge planners, and facilitate with service provider agencies to develop the community resources and services to ensure successful return of an individual to independent living within the community.

Project Out IL Transition Specialists will work with a team of professionals to assess eligibility of individuals for consideration to return to independent living in the community.

Eligibility

• Medicaid eligible or soon to be.
• Individual of any age.
• Disability or long term illness .
• Wyoming Resident.

Services Provided

• Housing assistance which may include:
  • First and last months rent.
  • Utility deposits.
  • Acquiring furniture.
  • Making a home accessible.
  • Transportation assistance (thru our Transportation Voucher Program.)
