Community Choices Waiver

We believe individuals with disabilities and chronic illnesses know best what services and supports they need to accomplish activities of daily living.

A variety of options are available for personal care under Medicaid’s Community Choices Waiver. 
WIL Case Managers help you understand the available options and assess your own support needs.

Agency Option

If you choose to use a home healthcare agency, WIL Case Managers help you coordinate services and advocate for your specific needs and preferences. Services may include:

  • Home-delivered meals
  • Lifeline
  • Light housekeeping
  • Personal care attendants
  • Non-medical transportation
  • Skilled nursing

Assisted Living Option

WIL Case Managers help you coordinate services and advocate for your needs and preferences within the assisted living environment of your choice.

Participant Directed Option

You recruit, hire and train your own direct service workers to meet your specific daily needs. WIL Case Managers help you determine your needs, find and hire direct service workers, and coordinate your own services, which may include:

  • Bathing
  • Personal grooming
  • Grocery shopping
  • Meal preparation
  • Therapeutic exercise
  • Medication management

Eligibility

Wyoming residents 19 years of age or older who meet financial and medical eligibility requirements established by the Wyoming Department of Health  (See a WIL Case Manager for additional details).
